FARTHER Outreach is an outreach program under the auspices of the Boynton Beach CDC, and dedicated to providing opportunities to individuals and their families in the inner cities of Palm Beach County.
In a team-oriented work environment, Team Members and participants work together to identify barriers keeping them from reaching their personal goals and develop reachable plans to achieve sustainable outcomes that enrich the quality of their lives.

All programs, activities and services are provided to the community and participants sign up on a voluntary basis regardless of their ethnicity, gender, creed, disability or national origin.
Offers resources, referrals and workshop services through collaborations with other agencies and businesses in the community to meet the needs of individuals we serve.
Need FOOD STAMPS, MEDICAL INSURANCE or CASH ASSISTANCE? We are a certified Dept. of Children & Families ACCESS Partner to the MyFlorida site. Services include completing the application process or recertifying benefits, submitting documents to DCF, and providing case status. All information is CONFIDENTIAL.
BUS PASS SERVICES
As an authorized bus pass agent for Palm Tran, you must be income eligible to purchase a 1-day bus pass for the Transportation Disadvantaged (TD) Program or 31-day bus pass at a discounted price. To complete an application with us, valid identification must be submitted at time of request (some examples are picture ID, proof of income, legal status, etc.). Call us for further details. CHAMPION SENIORS Program:
SAVING OUR CHAMPION SENIORS: Need assistance accessing resources? We advocate and guide you through each step of reaching your goals.
AGING IN PLACE Senior Care: Advocacy services to assist seniors to remain in their homes rather than enter institutional care by providing basic social services that address health concerns associated with senior isolationism, on-line shopping and FREE delivery, virtual social activities, LIHEAP and EHEAP utility payments, internet access and drive-by birthday celebrations.
CHAMPION SENIOR Mobile Library: Provides access to resources for mobility aids such as walkers, crutches, wheelchairs and life alert services are available to assist seniors in maintaining their independence at home and in their respective communities.
Are you in need of EYEGLASSES and have no funds to purchase them? Through collaboration with community partners, we coordinate referrals that lead to seeing things clearly again. For more details, give us a call.
HIV and AIDS Education & Outreach
Community outreach to bring awareness to the growing epidemic. Enjoy a family atmosphere with a movie and snacks KNOW YOUR STATUS, GET TESTED! (must sign up in advance).
